29 CRR 1960.8(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards from a falling automotive lift: In Building 20, Transporation, employees have been using an Northstar S-10AX two-post automotive lift for 2 1/2 years without having been inspected per the manufacturer's recommendations and was exposing them to struck-by and caught in-between hazards of a lift failure resulting in it prematurely falling to the ground. Among other methods, one feasible and acceptable abatement method to correct this hazard is to the follow the guidelines in the American National Standards Institute/Automotive Lift Institute ALOIM-2000 Section 5.5 wherein it specifies a minimum of a yearly inspection of the lift to ensure the safety and serviceability of the lift.ceability

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a fall hazard: At Building 25, Linen, on or about July 6, 2010, an employee was using a 20 ft fiberglass extension ladder with the ladder base on a sloped dirt embankment to the roof access point and did not stablize or secure the ladder to prevent the ladder from being displaced. Among other methods, one feasible and acceptable abatement method to correct this hazard is to follow 29CFR1910.25(d)(2)(i) and 29CFR1926.1053(b)(6) and the American National Standards Institute A14.5-1982, Section 9.3.4 Footing Support and Section 9.3.5 Top Support, to place the ladder base on a secure, level support surface and top support of the rails to prevent the displacement of the ladder.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards from motor vehicles: On VAMC Chiillicothe roads and parking lots, on or about July 6 and 7, 2010, two employees painting curbs and striping parking lots were exposed to a struck-by hazard from motor vehicles from the failure to wear the appropriate high visibility garment based on work risk hazards such as vehicle traffic and speeds encountered. Among other methods, one feasible and acceptable method of abatement to correct this hazard is to follow the American National Standards Institute/ISEA 107-2004 where in it specifies the performance class guidelines for high visibility garments to be worn based upon worker risk hazards such as vehicle traffic and speeds encountered.
