Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to slip and fall hazards while accessing and working in and around Rig # 123 due to several damaged sections of the walking working surface: (a)Xtreme Coil Drilling Corporation, Dry Creek #5-35, Weld #123, Greeley, CO 80631: On and before January 26, 2011, the exposing employer did not ensure that the walking surfaces near the ladder at the end of the cat walk was free from damage. This condition exposed employees to trip and fall hazards when accessing the area. Abatement Note: Among other methods, feasible and acceptable abatement methods to correct this hazard are to ensure proper supervision and daily walk through of the area to ensure that as sections of the grated deck are damaged they are quickly discovered and fixed. (b)Xtreme Coil Drilling Corporation, Dry Creek #5-35, Weld #123, Greeley, CO 80631: On and before January 26, 2011, the exposing employer did not ensure the raised hinges on several plates on top of the mud tanks did not create a tripping hazard. Abatement Note: Among other methods, feasible and acceptable abatement methods to correct this hazard are to ensure proper supervision and daily walk through of the area to ensure that as sections of the grates on top of the mud tanks are damaged they are quickly identified and fixed. Abatement Note: Abatement certification is required for this item (see enclosed "Sample Abatement-Certification Letter").-Certification
