OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crushing hazards associated with improperly maintained manual pipe tongs. (a) Xtreme Coil Drilling, Pergamos #3-11-7-60, Briggsdale, CO 80611: On and prior to January 22, 2013, both the "make up" and "break out" tongs were operating with missing latch assist springs (4). As a result, an employee received a serious hand injury on or about January 10, 2013 after the gate on one of the tongs unexpectedly opened as torque was applied.

(a) Xtreme Coil Drilling, Pergamos #3-11-7-60, Briggsdale, CO 80611: On or about January 22, 2013, the 7 bank control manifold was leaking hydraulic fluid onto the rig floor. Condition exposed employees to slips and/or falls from the same level or down the stairs leading from the rig floor.

29 CFR 1910.23(a)(8): Every floor hole into which persons can accidentally walk were not guarded: (a) Xtreme Coil Drilling, Pergamos #3-11-7-60, Briggsdale, CO 80611: On or about January 22, 2013, the employer did not ensure the floor hole in front of the vee-door measuring 29" X 12" was covered or guarded to prevent employees from accidentally steeping into. Condition exposed employees to cuts, contusions, and or laceration injuries. (B) Xtreme Coil Drilling, Pergamos #3-11-7-60, Briggsdale, CO 80611: On or about January 22, 2013, sections of missing wood rig mats exposed employees to accidentally steeping into the voids. Condition exposed employees to ankle and/or knee injuries.

This condition exposed employees to fall hazards of over 10 feet.Xtreme Coil Drilling was previously cited for a violation of this Occupational Safety and health standard or its equivalent standard CFR 29 CFR 1910.23( c )(1) which was contained in OSHA inspection number 456015, citation number 1, item number 3, issued on October 12, 2012, with respect to a workplace located at Pergamos #8-41-7-60, Rig #19.
