working behind a rear loading compacting truck that was operating in
reverse were exposed to
being struck by the truck.
a) 2463 Davis Court, Milford Township - On or about 16 December 2010, the
driver of the 1993 Ford Motor Trash Truck, being operated in reverse, did
not
know the locations of the employees who had been loading trash into the
rear of
the truck.
Abatement Note: Among other methods one feasible and acceptable method to
correct this
hazard would be to:
Ensure that employees follow the standards as outlined in the ANSI
Standard Z245.1-2008 for
Equipment Technology and Operations for Wastes and Recyclable
Materials---Mobile Wastes
and Recyclable Materials Collection, Transportation, and Compaction

Requirements.aste Equipment Technology Association Section 6.2.3.2 Driving
and riding
positions: When driving or riding in the vehicle.
1) Section 6.2.3.2 (f): When a vehicle is being operated in reverse
ensuring that the area behind
the vehicle is clear.
2) Section 6.2.3.2 (h): Making sure the locations of all co-workers are
known before operating
the vehicle in reverse.
3) Section 6.2.3.3: Use high visibility apparel.
4) Section 7.2.13: An external audible warning signal device (back-up
alarm) shall be provided
on all motor vehicles. In order to comply with this section either repair
and maintain the
manufactured installed horns and install and/or repair the reverse signal
alarms.
Abatement certification and documentation required within 10 days after
abatement date.  The
certification shall include a statement that abatement is complete, the
date and method of
abatement, and state that employees and their representatives were
informed of this abatement.
Abatement documentation shall include documents demonstrating that
abatement is complete,
such as evidence of the purchase or repair of equipment, photographic or
video evidence of
abatement or other written records.
