Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to falls and/or crushing injuries while riding on the rear of
a truck:
a) In the area of 1925 East Sawmill Road, Quakertown - On or about October
4,
2011, an employee was riding on the rear step of a 2001 Kenworth T800 Trash
Truck while the driver was operating the truck at a speed of approximately
25
miles per hour, and traveled approximately 1 mile, between stops.
Abatement Note: Among other methods one feasible and acceptable method to
correct these
hazards would be to: Follow the standards as outlined in the ANSI Standard
Z245.1-2008 for
Equipment Technology and Operations for Wastes and Recyclable Materials -
Mobile Wastes
and Recyclable Materials Collection, Transportation and Compaction
Equipment
- Safety
Requirements Equipment Technology Association Section 6.2.5.4a - Riding
steps, which states:
Ensuring that no persons ride on steps when the vehicle is exceeding 10
mph (17 kph),nor when
the distance traveled exceeds 0.2 mi (0.3 km).
Abatement certification and documentation required within 10 days after
abatement date.  The
certification shall include a statement that abatement is complete, the
date and method of
abatement, and state that employees and their representatives were
informed of this abatement.
Abatement documentation shall include documents demonstrating that
abatement is complete,
such as evidence of the purchase or repair of equipment, photographic or
video evidence of
abatement or other written records.

were exposed to being struck by vehicular traffic during non daylight
hours by not being visible
to motorists:
a) In the area of 1925 East Saw Mill Road, Quakertown - On or about October
4, 2011, employees were conducting waste collection work while operating
along
roadways during periods of darkness or non daylight hours without wearing
high
visiblity clothing.
Abatement Note: Among other methods one feasible and acceptable method to
correct these
hazards would be to: Follow the standards as outlined in the ANSI Standard
Z245.1-2008 for
Equipment Technology and Operations for Wastes and Recyclable Materials -
Mobile Wastes
and Recyclable Materials Collection, Transportation and Compaction
Equipment - Safety
Requirements Equipment Technology Association Section 6.2.3.3 - Use high
visibility apparel.
