North Carolina General Statute 95-129(1) of the Occupational Safety and
Health Act of North Carolina:  The
employer did not furnish each of his employees conditions of employment
and a place of employment free
from recognized hazards that were causing or likely to cause death or
serious physical harm to employees in
that employees were exposed to the hazard of a potential fire and/or
explosion due to combustible aluminum
dust:
a)Aluminum Polishing Department -  On August 23, 2010, a bulk aluminum
dust sample was
collected from the horizontal and vertical surfaces in the aluminum
polishing department.  The
results indicated that the aluminum dust was explosive and would support
an explosion hazard.
Aluminum dust was listed in the National Fire Protection Association
(NFPA) 484, Standards
for Combustible Metals, 2009 edition, as a combustible metal dust.  There
was aluminum dust
generated from the polishing process that was completed on aluminum gear
casings and
exhaust housings.  The fugitive aluminum dust ranged from one
thirty-second of an inch to
two inches on horizontal and vertical surfaces, including, but not limited
to, the following:  the
concrete wall, the polishing machines, the floor around the polishing
machines and the
conveyor system, the electrical disconnect boxes and electrical conduit
located behind the #3
polishing machine, the ventilation duct work, the work tables, etc.  It
was noted the aluminum
dust had accumulated not less than six inches in the first bend of the
intake ducts located at
each workstation at the polishing machines.  There were various
deficiencies in attempting
controls for the potential exposure to combustible aluminum dust,
including, but not limited to,
the following:  there had been no process hazard analysis completed for
the aluminum
polishing process; the housekeeping program was not effective in
eliminating the accumulation
of aluminum dust on horizontal and vertical surfaces in the work area;
fugitive dust control
measures were not in place; there was a lack of inspection, testing, and
maintenance
program
for the ventilation system; and the aluminum dust collection equipment and
duct work were not
grounded and bonded.

ABATEMENT NOTE:  Among other methods of abatement, one feasible and
acceptable
method to correct these hazards would be to follow the guidelines
established in the National
Fire Protection Association (NFPA) Standard - 484:  Standards for
Combustible Metals, 2009
edition.
