employees walking
through and working in the pipe conveyor/transfer lines were exposed to
the hazard of
being struck by or caught between moving pipes in instances such as but
not limited to
the following:
a.Final Finish at the south side of the skid east of OD Grinding
b.Final Finish at the north side of the skid east of OD Grinding
c.Final Finish at the north and south conveyor/transfer line south of the
Quality
Control work platform
d.Beveling at the north and south conveyor/transfer line north of the
Quality
Control work platform
e.Pre-Visual at the north and south conveyor/transfer line south of the
pre-Visual
Platform
f.Pre-Visual at the south side of the skid west of the Pre-Visual Platform
g.Skelp Plasma at he north and south conveyor/transfer line
h.Stencil Area at the north and south conveyor/transfer line
i.Beveling Area at the north and south conveyor/transfer line west of the
Beveler
and east of the exit door
j.Final Finish at the OD Grinding platform
Among other means, the following are feasible and acceptable methods to
correct
this hazard:
1.Install stairs and a walkway over the pipe conveyor/transfer lines.
2.Restrict Access to the pipe conveyor/transfer lines using guarding.
3.Develop, implement and require the use of lockout while employees are
performing work when they are in the pipe conveyor/transfer lines or on
platforms in the travel path of pipes.
