CCC Group Maintenance Shop:  On or about October 21, 2010, and at times
prior thereto,
an exit was obstructed by a chest freezer exposing employees to the
hazards of fire.

On or about October 21, 2010, and at times prior thereto, an acetylene
cylinder was stored in
an unprotected outside storage area at the CCC Group Maintainence Shop.
