Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazard of unintentional machine movement:
At the establishment, employees were exposed to a struck-by hazard from a
scissors lift due to
unplanned movement.  On or about 01/14/11, a fatality occurred involving a
Skyjack Scissors
Lift.  Skyjack Scissors Lift (model number SJIII 3219, serial number
220189498) unexpectedly
traveled in reverse direction and pinned an employee between the Skyjack
Scissors Lift and a
second Skyjack Scissors Lift (model number SJ 3220, serial number 62405).
One method to correct this apparent hazard is to implement lift start-up
and shut-down
procedures in accordance with the Operating Manual.

One method to correct this apparent hazard is to conduct pre-start
inspections (pre-operational
inspections).  Table 2.7 of the Operating Manual for the Skyjack Scissors
Lift (model number
SJIII 3219, serial number 22018949, manufactured in 2009) provides a
detailed operator's
checklist that meets the ANSI/SIA A92.6-2006 Section 7.3.3 Pre-start
inspection. Before use
each day or at the beginning of each shift, the aerial platform shall be
given a visual inspection
and a functional test including but not limited to the following:
(1)Operating and emergency controls.
(2)Safety devices.
(3)Air, hydraulic and fuel system leaks.
(4)Cables and wiring harness.
(5)Loose or missing parts.
(6)Tires and wheels.
(7)Placards, warning, control markings, and operating manual(s).
(8)Outriggers, stabilizers, and other structures.
(9)Guardrail system.
(10)Items specified by the manufacturer.

Schedule for Yearly, 3 months or 150 hours, and Daily checklist that meets
the ANSI/SIA
A92.6-2006 Section 7.3.1 Frequent Inspection and Section 7.3.2 Annual
inspection requirements.
The annual inspection date must also be included on the Annual Inspection
Record decal located
on the scissor assembly.
