employer did not
furnish employment and a place of employment which were free from
recognized hazards
that were causing or likely to cause death or serious harm in that
employees operating the
Cat 980 C loader were exposed to crushing injuries in the event of a
rollover or collision:
(a) In the Log Yard: On Jan. 25, 2011 and at times prior thereto, the seat
belt web in
the Cat 980C loader had been removed from the brackets.
Among other methods, one feasible and acceptable abatement method to
correct this hazard
would be to replace the missing seatbelt and mandate that all operators of
this and similar
equipment utilize the provided seatbelts and implement a training and
enforcement program
concerning the use of seatbelts.
Note: Abatement certification IS required for this item.
