hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to fall hazards:
Employees operating the manlift in the grain elevator were exposed to fall
hazards up to
120 feet.  The hoistway doors were not interlocked to prevent the
operating of the
manlift until the doors were closed.
Among other methods, one feasible and acceptable method to correct this
hazard is to repair the
existing hoistway door interlocks which will prevent the car to start
unless the door is in the
closed position as referenced in Rule 1501.2 of ASME A17.1, Safety Code
for Elevators and
Escalators, 1996 edition.
