Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees in
that employees were
exposed to burn hazards from explosion/fire hazards while using an
oxy/fuel torch:
Maintenance Yard: A flash back arrestor was not was not installed at the
inlet to the torch to
reduce or eliminate an explosion caused by a flash back incident.
On 10-17-11 an employee operating a cutting torch was injured during an
explosion/fire possibly
due to a burn back of the flame into the tip and mixing chamber, or
flashback event.
