Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to fall and struck by hazards:
(a) On December 3, 2010, at a site located at 4416 N. Clifton Street in
Chicago, Illinois 60646,
Suarez Roofing, Inc. provided a Garlock Equipment Company 300 lbs. ( A
frame) Hand Hoist
that was not equipped with approved counterweights.
Among other methods, feasible and acceptable means of abatement is to use
counterweight
material specified by the hoist manufacturer as described in the operating
instructions and
warning section of the Garlock Equipment Company 300 lbs (A frame) Hand
Hoist owner's
manual.
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
