Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which were free
from recognized
hazards that were causing or likely to cause death or serious physical
harm
to his employees in
that employees were exposed to a struck-by hazard:
(a)On or about 02/15/2011 - Main plant, the hooks from the overhead crane
being
used to lift a personnel platform and materials were missing the safety
latch.
Among other methods, one feasible and acceptable method to correct this
hazard
would be to comply with the requirements of the American Society of
Mechanical
Engineers (ASME), B30.10-2009 which states that a hook latch that is
inoperable
shall be repaired or replaced. ASME B30.23-1998, Section 10-2.10.6 Repairs
and
Modifications states that the attachment of a personnel lifting platform
shall be
a type that can be positively locked.ely
