Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which was free from
recognized hazards
that were causing or likely to cause death or serious physical harm to
employees in that
employees were exposed to the hazard of struck-by and caught-between when
working around
and working with rig derrick sections while rigging up:
This violation occurred on or about March 19, 2011, at Maness Gas Unit # 6
site where
employees were exposed to being struck-by and/or caught between rig
derrick sections
that were unloaded from a truck and set vertically on edge and
side-by-side.  One side
of each section was supported by a protruding spreader beam with a pin
connection
hinge
installed.  This caused the section to lean toward the other side which
was unsupported.
Employees walking and/or working between the sections were exposed to a
struck-by and
/or caught-between hazard in the event of a section falling to the
unsupported side as a
result of the unstable positioning of the sections.
Among other feasible methods to correct the identified hazard, comply with
the American
Petroleum Institute (API) Recommended Practice for Occupational Safety for
Oil Well
Drilling and Servicing Operations, API RP 54, recommendation to conduct a
review,
prior to commencing rig-up operations, of the planned arrangement of
equipment
placement to eliminate and/or minimize potentially hazardous conditions;
place the
derrick sections face down, when off loaded from the truck, to ensure that
they are in
a stable position.
Pursuant to 29 CFR 1903.19, within ten (10) calendar days, the employer
must submit
documentation describing the actions it is taking to ensure that the
placement of derrick
sections unloaded from trucks are set in a stable position to prevent them
from falling or
being knocked over.ed
