Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the condition(s) listed below: Reserve Drilling Inc., Rig 1, Loudamy 45 #2, Howard County, TX. For the period of time up to and including 06/17/11, the 3/4-inch emergency escape line had one clip in place where a minimum of three clips is required. API Recommended Practice 9B, Eleventh Edition, August 2002, Recommended Practice on Application, Care, and Use of Wire Rope for Oilfield Service, states under 3.7.4 "Refer to Table 2 and Table 3 for minimum number of clips, and torque required." Table 2 Attachment of Clips indicate a minimum of 4 clips for wire rope diameter 3/4-inch and Table 3 Attachment of Double Saddle Clips, indicated a minimum of 3 clips for wire rope diameter 3/4-inch. Feasible and acceptable abatement would include ensuring that the required minimum number of clips are being used to secure wire rope.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the struck-by hazards. Reserve Drilling Inc., Rig 1, Loudamy 45 #2, Howard County, TX. For the period of time up to and including 06/17/11, the high pressure cellar jet hose was not secured and the outer insulation had been compromised such that the inner coils were visible. API Recommended Practice 54, Third Edition, August 1999, Reaffirmed, March 2007, Recommended Practice for Occupational Safety for Oil and Gas Well Drilling and Servicing Operations, states under 9.13.4 "In normal operations, pumps, piping, hoses, valves, and other fittings shall not be operated at pressures greater than their rated working pressure and shall be maintained in good operating condition. Test pressures shall not exceed the design test pressure. Pumps, piping, hoses, and pressure relief devices shall be designed to meet the requirements of the operating conditions to be encountered." Feasible and acceptable abatement would include ensuring that high pressure hoses are maintained in good operating condition and that high pressure hoses are secured.
