29 CFR 1910.22(b)(1): Aisles and passageways were not kept clear and in good repair, with no obstruction across or in aisles that could create a hazard: On or about November 06, 2013, and at times prior thereto, employees were working on a rig site, Rig #3. The stair landing leading up to the top doghouse was bent up, rising up approximately 2.5 inches at some points. This created a trip hazard that could cause employees to stumble or fall down the stairs. The vertical height of the stairs measured more than 4 feet.

29 CFR 1910.219(c)(2)(i): All exposed part(s) of horizontal shafting seven (7) feet or less from floor or working platform were not protected by stationary casing(s) enclosing shafting completely or by trough(s) enclosing sides and top or sides and bottom of shafting: On or about November 06, 2013, and at times prior thereto, employees were working on a rig site, Rig #3. On mud pump #1, one of the rotating shafts was missing a cover. Employees were exposed to caught-by and/or amputation hazards.

29 CFR 1910.24(f): Rise height and tread width were not uniform throughout any flight of stairs including any foundation structure used as one or more treads of the stairs: On or about November 06, 2013, employees were working on Rig #3. The stairway leading up to pre mix pit had stair rungs # 3, 7, 8 and 9 that were damaged extensively which affected the riser height. This condition exposed employees to trip and/or fall hazards.

An accumulator cord had a damaged strain relief. This condition exposed employees to electrical hazards.
