Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to a struck-by hazard: a) On or about 02/24/11 in the fabrication welding areas bay 1 through 5, at least four hooks on the under hung hoist were missing the safety latches. Among other methods, one feasible and acceptable method to correct this hazard would be to comply with the requirements of the American Society of Mechanical Engineers, ASME B30.11-1998, Monorails and Under hung Cranes, Section 11-2.1.3.3, Adjustment, Repair, Replacements.
