Employees were working in the area of the media blasting machine that did not have a swinging gate. Employees were exposed to a 7 feet fall hazard.

At the main power panel that housed the main breakers for the stationary welding machines, one of the breakers was missing.

29 CFR 1910.304(g)(6)(vi)(C)(5): The employer did not ensure that exposed noncurrent-carrying metal parts of cord-and-plug connected equipment that may become energized was grounded when the equipment was used in damp or wet locations by employees standing on the ground.     a) For employees working at 5127 Bloomingdale Avenue in Tampa, as observed on or about 24 September, 2014. Employees were using 2 wire non grounded handheld lamps. the lamp cord was repaired and the polarity was reversed. Employee was in the process of changing the light bulb inside of one of the metal archways when contact was made with the metal casing that was energized and employee was electrocuted.

Employees were using a bench grinder with a work rest that was not properly adjusted closely to the wheel. The work rest measured approximately 3/16 inch from the wheel.

Employees were using a bench top grinder that did not have the adjustable tongue less than 1/4 inch opening above the abrasive wheel. The tongue guard was adjusted to approximately 1/2 inch above the abrasive wheel.

Employees were getting ice from the ice machine located in the north pipe shop break room area. The power supply cord for the ice machine located in the north pipe shop was missing strain relief.
