North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by falling load: a)mill house-the supporting jib structure for a chain fall, make not known, locally manufactured from portions of a bridge, was rusty, pitted, and not inspected. (Re: para16-2.1.3(9) ASME B30.16b-1995). One feasible means of abatement is to replace the locally manufactured support system with an authorized and inspected support system.

North Carolina General Statute 95-129(1) of the Occupational Safety and Health Act of North Carolina: The employer did not furnish each of his employees conditions of employment and a place of employment free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to being struck by a falling load: a)mill house-hook latches of a manual chain hoists, used to lift mill stones of approximately 1700 lbs, were not maintained in proper operation. (para 16-2.1.2(5), ASME B30/16b-1995). One feasible means of abatement is to install proper functional latches on all hooks on the hoists and conduct regular inspections.
