employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to struck by hazards while using hoist(s) that had not been
inspected to move
material:
a)801 Highway 31 South, Bay Minette, AL 36507; On or about May 17, 2011 and
at times prior thereto employees were exposed to struck by hazards while
using
a CMI overhead hoist that had not had a documented periodic inspection
performed at least annually. This hoist in the Hog area had a missing
safety latch
and the capacity of the hoist was not legible. A feasible and acceptable
method
to correct this hazard is to conduct frequent and periodic inspections of
the hoist
for missing or defective equipment in accordance with ASME B30.10-1999,
Hooks and ASME B30.16.-1998, Overhead Hoists (Underhung).
b)801 Highway 31 South, Bay Minette, AL 36507; On or about May 17, 2011 and
at times prior thereto employees were exposed to struck by hazards while
using
a Yale One Ton Hand Chain Hoist in the foil Slitter area that had not had a
documented periodic inspection performed at least annually. A feasible and
acceptable method to correct this hazard is to conduct frequent and
periodic
inspections of the hoist for missing or defective equipment in accordance
with
ASME B30.10-1999, Hooks and ASME B30.16.-1998, Overhead Hoists
(Underhung).
