Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that an employee
was exposed to being thrown from a bucket while riding in and standing in
the elevated bucket
of a front-end loader.
a) Ronald Road, Parsippany, NJ.
On or about July 12, 2011 an employee was being transported and elevated
in a bucket of a
front-end loader, thereby exposing the employee to being thrown from the
bucket itself and
struck by oncoming traffic.
Violation occurred on or about 07/12/2011.

hazard
is to follow the
owner operator manual located on Page 23 of the safety operation and
maintenance manual
excerpt,safety section engine starting."
"DO NOT ALLOW RIDERS ON THE MACHINE UNLESS THE MACHINE HAS AN
ADDITIONAL SEAT WITH A SEAT BELT."
