Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of propagation of a fire, deflagration, and/or
explosion outside the
area of the initial fire, deflagration, and/or explosion.
a) For recycling work performed with the ring mill: On or about January
10, 2012, and at times
prior thereto, the employer did not ensure that the ring mill used to
crush electronic equipment
was equipped with explosion venting, exposing employees operating and
working near the ring
mill to the hazards of fire and explosion.
b) The dust collector system was located inside the building without
explosion venting located
away from workers.
Feasible abatement methods to correct the hazard identified above include,
among
others:
implement National Fire Protection Association (NFPA) 68, 2002, Standard
on Explosion
Protection by Deflagration Venting for appropriate explosion venting.
Relief panels, windows
or other venting devices shall be designed to prevent reclosing after
relieving the explosion
pressure and shall be attached with retention cables or restrained by
equivalent means so that
they will not become projectiles upon relief. Implement standards for
suppression, containment
or inerting systems as described in NFPA 69 (2008) Standard on Explosion
Prevention Systems.
Locate the dust collector system (baghouse) outside the building.
Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date
of this citation, the
employer must submit documentation showing that it is in compliance with
the obligation,
including describing the steps that it is taking to ensure that employees
are protected from
explosions in the ring mill.

Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer
did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to the hazards of the escape of combustible electronic
equipment dust from
equipment and its accumulation in work areas, increasing the risk of fire,
deflagration, and/or
explosion.
For recycling work performed with the ring mill: On or about January 10,
2012, and at times
prior thereto, the employer did not ensure that the work area in and
around the ring mill was
kept free of escaping combustible dust which was likely to ignite or
explode, exposing
employees to the hazards of fire and explosion.
Among other feasible abatement methods to correct this hazard, one method
is to implement the
requirement of NFPA 654, 2006, Prevention of Fire and Dust Explosions from
the
Manufacturing, Processing, and Handling of Combustible Particulate Solids.
Employer must
implement control meausures to reduce and manage dust, including:
Appropriately designed and
maintained dust collection systems, minimization of the escape of dust
from equipment, and
inspection/cleaning at regular intervals.
Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date
of this citation, the
employer must submit documentation showing that it is in compliance with
the obligation,
including describing the steps that it is taking to ensure that
combustible dust is prevented from
escaping from the process.
