General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970. The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employee in that employees were exposed to fall hazards. a) On or about 10/03/11 at 150-161 Harriet Street, Woodbridge, NJ 07095: Employees were allowed to work in excess of 30 feet above ground level from a job- made work platform that was resting on the forks of a Lull 844C-42 (Lull Rough Terrain Forklift). The basket/platform was: 1) not approved. 2) missing guardrail system/fall protection. 3) not secured to the forks of the Lull. In addition, the employees working on the platform were not wearing a full body harness with shock absorbing lanyards. No fall protection was provided at heights of 30' from the ground below. ONE FEASIBLE AND ACCEPTABLE METHOD TO CORRECT THIS HAZARD, AMONG OTHER MEANS, IS TO FOLLOW THE GUIDELINES SET FORTH IN THE LULL/SKY TRAK OPERATOR AND SAFETY MANUAL, WHICH PROHIBITS THE USE OF A NON-APPROVED HOME MADE WORK PLATFORM.
