employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to an electrical hazard of electrocution:
On or about 10/06/11, a grounding jumper used to bond a phase conductor to
the supporting
tower's structural metal was not sufficiently inspected and maintained in
a manner that ensured
the jumper would grip and remain in position on the tower when connected.
A feasible and acceptable method in correcting this hazard is to comply
with requirement
outlined in ASTM F2249-2009 "Standard Specification for In-Service Test
Methods for
Temporary Grounding Jumper Assemblies Used on De-Energized Electric Power
Lines and
Equipment."
In accordance with 29 CFR 1903.19(d), abatement certification is required
for
this violation
(using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in
addition, documentation demonstrating that abatement is complete must be
included with
your certification.  This documentation may include, but is not limited
to, evidence of the
purchase or repair of the equipment, photographic or video evidence of
abatement, or other
written records.
