General-duty citation text
Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by and crushing hazards from loads being lifted using hooks that were missing safety latches. On or about 8/24/11, in the production area, the employer permitted use of hooks that were not equipped with safety latches to prevent displacement of the load being carried. One feasible means of abatement is to provide hooks equipped with safety latches intact per ASME B30.10-2009. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to metal racking collapse hazards: On or about 8/24/11, in the production area and maintenance shop, the employer did not install, inspect and load rate the metal racking systems used to store mold dies in accordance with ANSI MH16.1-2008. Among feasible means of abatement for the employer is to follow ANSI MH16.1-2008 for metal racking systems.
