furnish to each of his employees employment and a place of employment

recognized hazards that are causing or are likely to cause death or
serious physical harm to
his employees. The employees are exposed to the hazards of fire and
explosion.
On or about 10/31/2011:
(a) Employees were exposed to fire and explosion hazards from the dust
collection air-
material separator (dust collector) that is not protected by an explosion
venting system and is
located next to a walk way used by employees. Among other methods one
feasible and
acceptable method to correct this hazard is to install an explosion
venting system as
referenced in paragraph 7.1.2.1 of NFPA 654 Standard for the Prevention of
Fire and Dust
Explosions from the Manufacturing, Processing, and Handling of Combustible
Particulate
Solids, 2006 Edition
(b) Employees were exposed to dust and explosion hazards in that the dust
collection system
was not provided with a means of isolating equipment in the event of an
explosion in the air-
material separator to prevent propagation between pieces of equipment
connected by
ductwork. Among other methods, one feasible and acceptable method to
correct this hazard
is to install fast acting valve systems in accordance with NFPA 69,
Standard on Explosion
Prevention Systems. This paragraph is referenced in paragraph 7.1.4.1 of
NFPA 654,
Standard for the Prevention of Fire and Dust Explosions from the
Manufacturing,
Processing, and Handling of Combustible Particulate Solids, 2006 Edition.
In accordance with 29 CFR 1903.19(c), abatement certification is required
for this
violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
