Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish to each of his employees a place of employment which were free
from recognized
hazards that were causing or likely to cause death or serious physical
harm to his employees
in that the employees were exposed to defective hook latches:
A)  Star Iron Works, Inc. - On or about 8/17/11, 7H Overhead crane; twin
monorail 4000 lb
--- The latch on the crane's hook was deformed and did not return to the
closed position.
Condition potentially exposed personnel to a struck-by hazard.
ABATEMENT NOTE:
Among other methods, one feasible and acceptable methods to correct this
hazard would be
to replace the latch mechanism and comply with the requirement of the
American Society of
Mechanical Engineers, SAME B30.1.11-2004, Monorails and Underhung Cranes,
Section
11-2.1.4.4, Frequent Inspections and ASME B30.10 - 2009, Hooks Section
10-1.10.5
