OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to entanglement hazards from loose clothing and working operations that place the employee in close proximity to rotating machine components: a) Star Iron Works, Inc.; production area - On or about, 3/24/25, and at times prior thereto, employees were permitted to wear loose/unconfined clothing (i.e. flannel shirt) while in the process of machining a part on the Weiler E50 CNC lathe model #E50X2000 exposing employees to entanglement hazards. ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation. This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement. IN ADDITION, abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic, or video evidence or abatement or other written records.

Star Iron Works, Inc. - On or about 3/26/25, and at times prior thereto, the company did not record the information in Column F of the OSHA 300 Injury and Illness log with all the information required by the regulation. This occurred on Case #3 on the 2025 OSHA 300 Injury and Illness log.
