employer did not furnish employment
and a place of employment which were free from recognized hazards that
were causing or likely to cause
death or serious physical harm to employees in that employees were exposed
to crush and struck-by hazards
associated with the unknown weight and capacity of the below-the-hook
lifting device:
a) Shipping/Receiving Bay, Below-The-Hook Lifting Device: Spreader Bar
Assembly - Neither the
weight nor the capacity of the assembly were known.  The device was
suspended from the monorail
crane with a 3-ton rated capacity.  On or about 10-13-2011.
AMONG OTHER METHODS, ONE FEASIBLE AND ACCEPTABLE ABATEMENT METHOD
TO CORRECT THIS HAZARD IS: To follow and implement the applicable
components of
ASME (American Society of Mechanical Engineers) consensus document
B30.20-1999 including
but not limited to:
1- Have the device structurally evaluated by a person qualified to make
such an evaluation.
2- Have the evaluation determine the maximum safe lifting capacity of the
device.
3- Using a certified scale, determine the actual weight of the device
assembly.
4- Conduct a load test the device assembly at 125% of its determined
maximum rated capacity.
5- Label the device with its established list of components, its total
weight, and the maximum safe
lifting capacity of the device.
CERTIFICATION OF ABATEMENT OF THIS VIOLATION IS REQUIRED.
