29 CFR 1910.212(a)(1):     One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:      a) Press Shop:  Employees were operating a Cincinnati Press Brake (#16, Serial No. 43635) with improperly installed light curtains, exposing employees to the point of operation. The right side of the two-sided light curtain was not working, observed on or about 5/16/12.

Buckets and other material were stored in front of the electrical panels, impeding access to them, observed on or about 5/16/12.
