Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The
employer did not
furnish employment and a place of employment which were free from
recognized hazards that
were causing or likely to cause death or serious physical harm to
employees in that employees
were exposed to being struck by concrete forms:
WORKSITE: The employer neither provided appropriate and specific training
and instruction
prior to allowing employees to perform the "task", dismantling the PERI
CB240 climbing
scaffolds/concrete forms, nor conducted visual observation while
performing the task.
On or about 10/19/2011, a section of a PERI CB240 climbing scaffold
/concrete form, which
was not properly secured in the upright position fell and struck one of
employees while
performing the task due to improper removal of a part. The employee
sustained serious injuries
from the incident.
Among others, one feasible and acceptable method to correct this hazard is
to:
1. Develop and implement written step-by-step procedures for the proper
dismantling of the
scaffold/concrete form.
2. Train employees in the safe work practices and procedures.
3. Utilize other engineering feasibilities.ete
