Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The
employer did not furnish
employment and a place of employment which were free from recognized
hazards that were
causing or likely to cause death or serious physical harm to employees
working from elevated
heights of oil-well service rig derrick.
On or about January 17, 2012 and times prior to, at Reaner #1, an employee
worked from the
tubing board where an emergency escape device was not installed and
available for use. The
employee working from the tubing board was exposed to a potential fall or
severe burns
associated with, and in the event of a fire or explosion at the oil-well
service rig platform.
One feasible and acceptable abatement method to correct this hazard would
be to ensure that an
auxiliary means of escape is installed and ready for use prior to
personnel working in the derrick
in accordance with Recommended Practice for Occupational Safety for Oil
and Gas Well Drilling
and Servicing Operations API 54, Third Edition, August 1999, Section
6.10.1, Auxiliary
Escape: "On all land rigs, the derrick or mast shall have an auxiliary
means of escape installed
prior to personnel working in the derrick.
