On or about 11/10/2011, employees were exposed to struck by related
hazards when
compact track loader was not operated in accordance with the operation and
maintenance
manual.
Among other methods, a feasible means to eliminate or reduce this hazard
is to follow
the operation and maintenance manual that instructs the operator to
constantly survey the
area around the machine and to prohibit employees to operated heavy
equipment while
in close proximity to other employees.
