Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees in that employees were exposed to struck-by hazards: Throughout the workplace: On December 12, 2011, employees engaged in material overhead lifting activities were exposed to struck-by hazards from suspended loads due to overhead crane hooks with visible defects not being removed from service. Overhead crane hooks in the following locations contained visible defects, such as, but not limited to, damaged/malfunctioning latches that did not close the hook's throat: a.Stage 1 Zone-Southwest Area of Shop: 1 Ton Overhead Crane, Pedestal Mount, b.South end of Pit 2 Area, West Center Area of Shop: 5 Ton Overhead Crane, c.North end of Pit 2 Area, West Center Area of Shop: 5 Ton Overhead Crane. Among other methods, one feasible means of abatement would be to remove from service those overhead crane hooks with visible defects in accordance with Section 10-1.10.5 Removal Criteria of ASME Standard B30.10-2009: "Hooks- Safety Standard for Cableways, Cranes, Derricks, Hoists, Hooks, Jacks and Slings".
