Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish to each of his employees a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to his employees in that employees were exposed to the hazard of inhalation of ammonia: (a)On or about January 25, 2012 - Ammonia Refrigeration Machine Room, entrance door to the machine room was not provided with a permanent sign reading " Machinery Room - Authorized Personnel Only", as required in ASHRAE Standard 15-2001, Section 11.2.4. (b) On or about January 25, 2012 - Ammonia Refrigeration Machine Room, the ammonia detector located in the machine room was set at 250 ppm to actuate the alarm and mechanical ventilation. This concentration was above the TLV-TWA for ammonia and presented a health hazard (inhalation) not protecting the employees as required in ASHRAE Standard 15 - 2001, Section 8.11.2.1. (c) On or about January 25, 2012 - Ammonia Refrigeration Machine Room, a visual and audible alarm was not provided in the ammonia machine room as required in ASHRAE Standard 15- 2001, Section 8.11.2.1. (d) On or about January 25, 2012 - Ammonia Refrigeration Machine Room, emergency remote controls were not provided outside the machine room for the purpose of shutting down the equipment in an emergency as required in ASHRAE Standard 15 - 2001, Section 8.12(i).
