Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing, or likely to cause, death or serious physical harm to employees in that employees were exposed to a struck-by hazard: Employees were exposed to a struck-by hazard from falling "I" Beams during the construction of a natural gas gathering facility. a.Link Belt crawler crane and hydraulic mobile cranes were lifting "I" beams with a single Elephant Eye Beam Clamp. The top flange of the "I" Beam was held by the top opening of the jaws in the Clamp to lift the "I" beam. b.Employees were not provided a spreader bar with two Elephant Eye Beam Clamps to pick up long pieces of material. c.Employee did not receive any training prior to rigging this "I" beam. Among other methods, one feasible and acceptable abatement method to correct this hazard is: 1.Implement a policy that requires a spreader bar be used with two Elephant Eye Beam Clamps (one at each end) when picking up a long "I" Beam. 2.Implement a policy that requires that all riggers be trained in the correct procedures to follow when rigging a load to be lifted into place by a crane.
