29 CFR 1926.405(a)(2)(ii)(E): Lamps used in temporary wiring for general illumination were not protected from accidental contact or breakage: At this construction site, in tool shed #2, a hand-lamp was being utilized for illumination and did not have a cover to protect it from possible breakage. Employees were exposed to possible electrical shock and burns from the exposed bulb.

The extension cord was not protected from being run over by equipment.
