furnish employment and a place of
employment which were free from recognized hazards that were causing or
likely to cause death or serious
physical harm to employees:

Location:Injection Molding Production Department
Equipment:200 Ton Van Dorn Injection Molding Machine #13, Model
#200-RS-14F, Serial #200-
RS-14F-1998
All Other Similar Injection Molding Machines Identified by the Employer as
Machine
Numbers 2, 3, 4, 5, 6, 7, 8, 9, 10, 11, 14,15, 17 and 30
Condition:Employees were exposed to severe burn hazard from scalding water
when changing the
water filter on the machine from failure to regulate water flow through
the oil cooler
(oil heat exchanger) to maintain safe hydraulic oil temperature during
operation.
Among other methods, one feasible and acceptable abatement method to
correct this hazard includes, but is not
limited to:
A.Install and equip the machine with an oil temperature alarm, light, or
bell and pump shut-down
device to monitor and maintain safe hydraulic oil temperature during
operation.
B.Follow and comply with the machine manufacturer's instructions and
recommended work
practices.
C.Install a temperature device to monitor and maintain water temperature
during operation in
accordance with Manufacturer's Specifications ASTM D1785 and ASTM D1784
for the
Schedule 80 PVC pipe and ASTM F1970 UPVC True Union Ball Valve.
D.Follow and comply with the ball valve manufacturer's instructions and
recommended work
practices.
E.Schedule and conduct preventative maintenance on a regular basis to
ensure proper operation
of the Legend Valve UPVC True Union Ball Valve.
F.Develop and implement written standard operating procedure for changing
the water filter to
ensure safe work practices.
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
