likely to cause death or serious
physical harm to employees:

Condition:Employees were exposed to crush/struck-by hazards from
collapsing steel storage racks
that were accessed by a forklift and were not secured to the floor.
Among other methods, one feasible and acceptable method to correct this
hazard is to follow ANSI
MH161.1:2008, 1.4.7 Specification For The Design and Utilization of
Industrial Steel Storage Racks. All rack
columns shall be anchored to the floor with anchor bolts capable of
resisting the forces caused by the
horizontal and vertical loads on the rack.
Abatement documentation (photographs, receipts, copies of programs,
training certification, etc.)
is required for this item.
