Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:04/01/14 Location:North Side of Building Equipment:Carelift All-Terrain Forklift, Model #ZB8038-44, Serial #B803800778C Condition:The forklift's seatbelt was not worn by the operators because the buckle was damaged and would not stay latched. Among other methods, one feasible and acceptable method to correct this hazard, install and utilize a restraint system in accordance with the current version of American National Standards Institute (ANSI) B56.1, "Safety Standard for Powered Industrial Trucks," and American Society of Mechanical Engineers (ASME) B56.1- 2000, "Safety Standard for Low Lift and High Lift Trucks."

Labor and Employment Article, Section 5-104(a): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees: (a)Date of Violation:On or About 04/01/14 Location:Third Floor of House Condition:Employees were exposed to the hazard of being crushed/struck by collapsing roof trusses due to inadequate bracing techniques. Among other methods, one feasible and acceptable abatement method to correct this hazard is to brace the trusses in accordance with the Wood Truss Council of America recommendations per BCSI B-1 and BCSI B-2. Abatement documentation (photographs, receipts, copies of programs, training certification, etc.) is required for this item.
