29 CFR 1926.350(f)(3): Hose(s) in use carrying acetylene, oxygen, natural or manufactured fuel gas, or any or substance which could ignite or enter into combustion, or be in any way harmful to employees, was not inspected at the beginning of each work shift. Defective hoses were not removed from service: a) On or about April 19, 2011, employees were exposed to injury in that, defective oxy/acetylene hoses were in use at the site (south building). Both hoses were damaged near the connection point at the torch and had not been effectively inspected before use on that shift.
