1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder.    (a)Complete Roofing Services, LLC @ 5407 Fox Run Blvd, Frederick, CO 80530: On and before May 9, 2011, the employer did not ensure that employees were utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface. Employees were using an extension ladder for access to the roof that did not extend 3 feet past the roof line. Employees were exposed to a fall of approximately 13' while attempting to use the ladder.
