1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m)above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder    (a)Precision Roofing and Design, Inc. @ 5122 2nd Street, Boulder, CO 80301: On and  before May 14th, 2011, the employer did not ensure that an employee were utilizing a portable ladder in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface.  An employee was using a portable extension-ladder for access to the roof that was not extended three feet above the roof line. An employee was exposed to a fall of approximately 21' while attempting to use the ladder.
