29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor was an alternative fall protection measure provided.    (a) Precision Roofing and Design, Inc. @ 5122 2nd Street, Boulder, CO 80301: On and before May 14th, 2011, the employer did not ensure that fall protection systems were utilized by an employee engaged in roofing activities while working at the unprotected edges of a roof on a single-family residential housing. Employees were exposed to fall hazards of approximately 21'.

1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m)above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder    (a)Precision Roofing and Design, Inc. @ 5122 2nd Street, Boulder, CO 80301: On and  before May 14th, 2011, the employer did not ensure that an employee were utilizing a portable ladder in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface.  An employee was using a portable extension-ladder for access to the roof that was not extended three feet above the roof line. An employee was exposed to a fall of approximately 21' while attempting to use the ladder.
