29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b):    a)  Precision Roofing and Design, Inc. at 2128A Hecla Drive, Louisville, CO 80027:  On or about June 8, 2013, the employer did not ensure that employees were using fall protection when installing metal drip edging on a residential roof with a 4 in 12 pitch, that measured 10 feet x 18 feet.  Condition exposed employees to fall hazards.       Precision Roofing and Design, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(13), which was contained in OSHA inspection number 29563, citation number 1, item 1, issued October 6, 2011, and became a final order on November 16, 2011, with respect to a workplace located at 5122 2nd Street, Boulder, CO 80304.

29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:    a)  Precision Roofing and Design, Inc. at 2128A Hecla Drive, Louisville, CO 80027:  On or about June 8, 2013, the employer did not ensure that an employee wore a hard hat when handling metal roofing material up to another employee on the roof.  Condition exposed this employee to struck by hazards.
