29 CFR 1910.23(a)(9): Each floor hole, into which persons could not accidentally walk, was not protected by a cover that leaves no openings more than 1 inch wide. On Machine #743, there was a floor hole on the catwalk, that was approximately 4 inches X 4 inches wide. No abatement certification or documentation is required for this item in accordance with 29 CFR 1903.19(c).

29 CFR 1910.132(a): \r\n\r\nProtective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: a. A method of fall protection was not used for Operators climbing on top of the machines, when cleaning off the machine, due to the changing of shot size. b. Head protection was not provided for employees working under a platform in order to setup, clean up or adjust Machine No. 582, and head protection was not provided when employees had to climb inside of shot peening machines to set them up. Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).\r\n

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage on electrical cords that could have exposed an employee to injury, and the defective or damaged item was not removed from service: On Machine #771, the outer insulation for the power cord, attached to the foot pedal, had pulled away, exposing the innter insulated wires and it was not removed from service. No abatement certification or documentation is required for this tiem.
