29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from fallingor flying objects, or from electrical shock and burns, were not protected by protective helmets: (a) D & D Roofing, Inc @ 40 South Boulder Circle, Boulder, CO 80303: On or before 6/7/11, the employer did not ensure that an employee on the worksite where flying and/or falling objects were present was equipped with head protection. Debris was thrown from the roof to a work vehicle below near where an employee was located to ensure the area was kept secure from tenants. A lack of hard hat exposed the employee to lacerations and/or bruises.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: (a)D & D Roofing, Inc @ 40 South Boulder Circle, Boulder, CO 80303: On or before 6/7/11, the employer did not ensure that employees working on a residential roof were protected from a fall. Four employees were observed performing tear-off activities on a 3:12 pitch roof. Each employee wore a body harness and lanyard, but did not utilize the installed rope grab system. The roof was measured from ground to eve at the approximate location where employees dropped shingles to the truck below. This condition exposed employees to an approximate fall distance of 18ft. D&D Roofing, Inc was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection 313717951, citation number 1, item number 1, issued on 3/22/10, with respect to a workplace located at 1745 Allison St., Denver, CO 80232. D&D Roofing, Inc was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard 29 CFR 1926.501(b)(13) which was contained in OSHA Inspection 313720062, citation number 2, item number 1, issued on 7/01/10, with respect to a workplace located at 1801 & 1891 S. Allison St., Lakewood, CO 80232.
