29 CFR 1926.502(b)(2): Employers shall provide and install all fall protective systems required by this subpart for an employee, and shall comply with all other pertinent requirments of this subpart before employee's begin work that necessitates the fall protection: (a) B&M Roofing of Colorado, Inc. @ 150 Miller Ranch Rd., Edwards, CO 81632: On and before May 8th, 2010, the exposing employer did not ensure that fall protection equipment assigned or issued, was suitable and complied with the applicable provisions of the standard. An employee was using a positioning system while conducting roofing operations. A rope grab system was used through the metal roof where the employee tied it off to the side D-rings for fall protection. The condition exposed the employee to an 8 foot fall hazard while working on a secondary building on the main roof of the Colorado Mountain College.

29 CFR 1926.1053(b)(1): Because of the portable ladder's length, the ladder side rails did not extend to at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access and the ladder was not secured at its top to a rigid support, and a grasping device, such as a grabrail, was not provided to assist the employees in mounting and dismounting the ladder (a) B&M Roofing of Colorado, Inc. @ 150 Miller Ranch Rd., Edwards, CO 81632: On and before June 6th, 2011, the employer did not ensure that an employee was utilizing portable ladders in such a way that enabled them to access and egress from work areas using ladders that extended 3' above the walking working surface. An employee was using a portable extension-ladder for access to the roof. The employees was exposed to a fall of approximately 8' while attempting to use the ladder.
