29 CFR 1910.23(a)(2): Every ladderway floor opening or platform shall be guarded by a standard railing with standardtoeboard on all exposed sides (except at entrance to opening), with the passage through the railing either provided with a swinging gate or so offsetthat a person cannot walk directly into the opening:    a.    In the small dump room, there was a hole to a ladder way opening where employees could enter the basement which was not properly guarded from employees accidently falling into the hole there by exposing employees to a seven foot fall hazard.

29 CFR 1910.23(c)(1):     Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides,        a.     On the top of block dust house system where employees perform maintenance,  clean the dust collector, and check temperature of the grain, the employer did not provide a means of fall protection thereby exposing employees to an 8 foot fall hazard.      b.     On the fourth level of the Old House where doors are opened to load or unload materials, the employer failed to install fall protection such as, but not limited to a guardrail system thereby exposing employees to a 100 foot fall hazard.    c.      On the third level of the Old House where doors are opened to load or unload materials, the employer failed to install fall protection such as, but not limited to a guardrail system thereby exposing employees to a 70 foot fall hazard.     d.      On the second level of the Old House where doors are opened to load and unload materials, the employer failed to install a fall protection system such as, but not limited to a guardrail system thereby exposing employees to a 50 foot fall hazard.    e.      On the first level of the Old House where doors are opened to load and unload materials, the employer failed to install a fall protection system such as, but not limited to a guardrail system thereby exposing employees to a 30 foot fall hazard.

29 CFR 1910.23(e)(1): A standard railing shall consist of top rail, intermediate rail, and posts, and shall have avertical height of 42 inches nominal from upper surface of top rail to floor, platform, runway, or ramp level. The top rail shall be smooth-surfacedthroughout the length of the railing. The intermediate rail shall be approximately halfway between the top rail and the floor, platform, runway, orramp. The ends of the rails shall not overhang the terminal posts except where such overhang does not constitute a projection hazard:    a. In the old house power room where employees entered and exited the basement, the employer did not provide a midrail form the stairway thereby exposing employees to a fall hazard.    b. In the main control room where employees gained access to the basement, the stairs were not equipped with a midrail.    c. On the warehouse dock where employees walked to load and unload trucks, the employer did not install midrail thereby exposing employees to a fall hazard.    d. In the mezzanine stairway from the ground to the basement, the employer failed to install midrail thereby exposing employees to a trip/fall hazard.    e. In the small dump room where employees entered the basement from a ladder way, the employer did not install a midrail.

29 CFR 1910.305(e)(1): Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations shall be installed so as to prevent moisture or water from entering and accumulating within the enclosures and shall be mounted so there is at least 6.35-mm (0.25-in.) airspace between the enclosure and the wall or other supporting surface. However, nonmetallic enclosures may be installed without the airspace on a concrete, masonry, tile, or similar surface. The enclosures shall be weatherproof in wet locations:    a.
