OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to explosion, deflagration and fire hazards associated with the ignition of combustible grain dust(s): a. Coshocton Grain Company, Coshocton Ohio, 475 Browns Lane facility: On or about August 14, 2014, material handling equipment, such as but not limited to, the dumper belt (the enclosed conveyor belt which moves grain from the blockhouse pit into the grain elevator) was not provided with adequate controls to minimize ignition risk for combustible grain dust generated and contained within the equipment. Conveyor belts were not provided with continuous belt alignment devices and bearing thermal sensors which would minimize the risk for ignition of combustible grain dust accumulations and airborne dust inside the conveyor belt enclosures.

29 CFR 1910.272(j)(1): Housekeeping. The employer shall develop and implement a written housekeeping program that establishes the frequency and method(s) determined best to reduce accumulations of fugitive grain dust on ledges, floors, equipment, and other exposed surfaces. Coshocton Grain Company, Coshocton Ohio, 475 Browns Lane facility: On or about August 14, 2014, the employer failed to develop and implement a written housekeeping program that specifically addresses conveyor belt enclosures, such as but not limited to the dumper belt enclosure. The employers written program does not address this equipment and the equipment is not part of routine housekeeping to reduce risks from the accumulation of combustible grain dusts. The Coshocton Grain Company was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 63114, citation number 1, item number 2a and was affirmed as a final order on August 17, 2011, with respect to a workplace located at 475 Browns Lane, Coshocton Ohio facility.

Equipment, wiring methods, and installations of equipment in hazardous (classified) locations shall be intrinsically safe, approved for the hazardous (classified) location, or safe for the hazardous (classified) location. Coshocton Grain Company, 475 Browns Lane, Coshocton, Ohio workplace: On or about August 13, 2014, an unrated trough style pull box located on the wall near the opening for the dumper belt conveyor tunnel into the first floor elevator room was not rated and approved for a Class II Division 2 hazardous location, was not intrinsically safe, or otherwise safe for a Class II Division 2 location. The Coshocton Grain Company was previously cited for a violation of this occupational safety and health standard which was contained in OSHA inspection number 63114, citation number 1, item number 3 and was affirmed as a final order on August 17, 2011, with respect to a workplace located at 475 Browns Lane, Coshocton, Ohio.
